The disability classification certificate

The certificate opens the door to your child’s rights, benefits and habilitation and rehabilitation services. The file is submitted to the DGASPC in the sector or county where you live.

IN BRIEF

What you need to know in ten seconds

Where to submit the file
At the DGASPC office in your sector or county of residence.

Who submits it
The parents or the child’s legal representative.

The child’s presence
Required at the assessment. The exception is children who cannot travel, for whom a home assessment can be requested, based on an application.

Renewal
The re-assessment application must be submitted at least 60 days before the previous certificate expires.

THE FILE

What documents you need

They are all submitted together, for the child’s comprehensive assessment.

The application and identity documents

The starting point of any file, whether for a first classification or a re-assessment.

  • Standard application for comprehensive assessment and disability classification
  • Copy of the child’s birth certificate or identity document
  • Copies of the identity documents of the parents or legal representative
  • Copy of the document proving that the person signing the application is the child’s legal representative — court ruling, Child Protection Commission decision, power of attorney

Social inquiry and income

Documents that come from the town hall and the tax administration.

  • Social inquiry and environmental factors as an annex, from SPAS or the town hall of residence — valid for 3 to 6 months
  • Income certificates of the parents, legal representatives, professional foster carers or other relatives who care for the child (grandparents, aunts). If they have no income, they request a certificate from ANAF or SPIT stating this

Medical documents

The most sensitive part of the file, because each document has its own validity period.

  • Summary medical form — issued free of charge by the family doctor, following the annual health check, and by the treating physician. Valid for 1 year
  • Type A5 medical certificate — issued free of charge by the paediatric specialist, depending on the child’s condition. It includes the full diagnosis — main illness, associated conditions and other illnesses — and the ICD-10 disease code
  • Recent additional medical documents stating the clinical form of the illness, stage, complications and medical recommendations: medical letters, discharge notes, investigation results

Psychological and psycho-pedagogical assessments

Two forms completed by different specialists.

  • Psychological assessment form — completed by a psychologist licensed to practise, certified in clinical psychology, in the public or private sector. Valid for a maximum of 3 months
  • Psycho-pedagogical form — completed by the teacher who works directly with the child in the school

Reassessment only

Added to the file when the certificate is renewed.

  • Copy of the current disability degree certificate
  • Copy of the current school and vocational guidance certificate and of the individualised service plan, where applicable

PSYCHOLOGICAL ASSESSMENT

When it is mandatory

The rule depends on the diagnosis and on whether it is a first classification or a reclassification.

At first classification

Psychological assessment is mandatory. For children with physical, somatic or sensory disabilities and rare diseases without mental disorders, the assessment is accompanied by one or two psychological counselling sessions, on the recommendation of the psychologist who assessed the child.

At reclassification, for cancer

For children with intellectual, mental and associated disabilities, as well as for children with HIV/AIDS and cancer, psychological assessment is mandatory at every reclassification of disability degree.

At reclassification, in other cases

For children with physical, somatic or sensory disabilities and rare diseases without mental disorders, psychological assessment is not mandatory at reclassification. If it is considered necessary, it is done only on the recommendation of the family doctor or the specialist.

Validity periods

Most files are sent back because of an expired document. Check them all before you submit.

3–6 months

Social inquiry. It cannot be older than 6 months, nor more recent than 3 months.

1 year

Summary medical form, from the family doctor and the attending physician.

3 months – 4 years

Medical certificate type A5. The exact period is set and written on the certificate by the doctor who issues it.

Maximum 3 months

Psychological assessment form.

APPOINTMENTS AND REASSESSMENT

What you need to follow

When you request a re-evaluation

The appointment or request for the child’s re-evaluation must be made at least 60 days before the previous certificate expires. The date for submitting the application is set out in the contract with the family.

A request for re-evaluation can be made at any time if the circumstances for which the certificate was issued have changed.

Attendance at the Serviciul de Evaluare Complexă

The child’s presence at the assessment is mandatory. The only exceptions are children who cannot be moved, for whom a home assessment can be requested on the basis of an application.

If, for objective reasons, you cannot attend at the date and time set, please call at least 24 hours in advance. Failure to attend will result in the assessment being rescheduled.
